What the RAMS sign-off sheet contains
The reference and the revision
Both sit at the top, side by side. Signatures against revision 1 cover nothing once the site is working to revision 3.
A column for what they asked
On by default. A sheet with nowhere to write a question records attendance, not a briefing. Untick it and the signature box takes the space.
Nothing is pre-written
No stock hazards, no ready-made control measures, no legislation quoted. The hazards on your job are yours — a template that guesses them says an assessment happened when it did not.

A paper sign-off sheet records a signature, not a briefing
That is not a criticism of paper — it is what paper is. The trouble starts when the sheet is asked to show that the gang was briefed on the method they are actually working to.
The RAMS moves on, the sheet does not
The method gets revised on Tuesday. The signatures collected on Monday stay in the folder, looking exactly as valid as they did before.
A signature is not a time
The date column says Monday 7am. Nothing says it was Monday 7am, or that the RAMS was open in front of anyone when the pen went round.
It lives in a lever-arch file
The sheet goes into the site folder and is looked at twice: when the principal contractor asks for it, and after something has gone wrong.
The same briefing, but
You print a QR code on the front of the RAMS, on the hoarding or on the welfare unit door. Whoever is briefed scans it, reads the method on their own phone, and confirms.
Time-stamped on our side
The entry carries the time of the scan, not the time someone wrote in a box. And it is attached to the document you published — revise it, and you can see who has confirmed the current version and who is still on the old one.
Nothing to install, nothing to collect
No app and no account for the person scanning. Nothing to gather at the end of the job either: the confirmations are already in order, readable from anywhere, exportable in full.
Read it the night before, from the office
A subcontractor who opens the method statement the evening before is a better outcome than one who skims it at the gate, so the location check is off unless you turn it on. When you do want the confirmation to happen on site, switch it on — and a back-up PIN covers the basement where nothing gets a signal.
It records that a named person confirmed they were briefed, and when. It is not a compliance document and we make no claim that it is one.
